Children's Complex Care Coalition of North Carolina (4CNC) (2023-2024)

Children with medical complexity (CMC) have high rates of health care utilization, require frequent hospitalization and experience many unique challenges in both the transition from the hospital after discharge and in daily life at home. This project team sought to better understand the perspectives of CMC caregivers on barriers and facilitators to good days at home and investigate the hospital-to-home transitional care management process. Several gaps in the systems supporting CMC and their families in care transitions and at home were identified, along with opportunities for improvement.
